Serving as Patient Coordinator
One of the most meaningful parts of my role is helping patients stay connected to their care. I have seen how easy it can be for someone to feel overwhelmed by the healthcare system. Missed appointments, confusion about next steps, or challenges at home can create barriers to healthcare. I help patients schedule follow-up visits, answer questions, and navigate these challenges in a way that makes them feel manageable. This could range from having a simple conversation that builds trust to working through more complex situations that involve ongoing support.
Through this work, I have come to better understand how social determinants of health shape health outcomes. Resources such as access to food, housing, and support systems play a major role in a person’s ability to stay healthy. I try to approach every interaction with empathy and patience, knowing that each person’s situation is different. I want patients to feel supported and confident in their ability to be engaged with their healthcare.
This experience has strengthened my communication skills and deepened my commitment to public health. It has shown me the importance of meeting people where they are and working alongside them to find solutions. Most importantly, it has reminded me that even small actions can make a meaningful difference in someone’s ability to access and navigate care.
