Position Summary: SFDPH's Value-Based Care Coordinators - Quality Improvement NHC members will support quality improvement initiatives across primary, specialty and inpatient units. They will help with telephone outreach, data analysis, and develop standard work to help quality improvement efforts and health outcomes.

The NHC members will improve health outcomes in the San Francisco community by supporting vaccination efforts, preventative screening outreach, referral to resources, and health education. They will aid in improving cancer screening rates, improving chronic disease control, and improving child and flu immunization rates through direct telephone outreach to patients.

About the Organization: The San Francisco Department of Public Health is a safety net healthcare organization that serves low income and vulnerable populations. Since the pandemic, the SFDPH has continued its efforts to slow the spread of viruses, protect vulnerable individuals, and prevent the healthcare system in the city from being overwhelmed.

The San Francisco Department of Public Health (SFDPH) strives to achieve its mission of protecting the health and safety of all San Franciscans through the work of two Divisions - the San Francisco Health Network (SFHN) and Population Health and Prevention. The SF Health Network is the City's health system and has locations throughout the City including San Francisco General Hospital Medical Center, Laguna Honda Hospital and over 12 primary care clinics that provide care for 64,000 active patients. SFHN patients are all publicly insured or uninsured, most are low -income and may experience low health literacy. The member will be interacting with patients and staff of many different backgrounds.

Major Duties and Responsibilities

  • Creating and developing communications related to outreach campaigns

  • Developing and providing health education for the community

  • Gathering data on critical issues impacting the community

  • Providing education to clients and referring them to providers

Knowledge Required for the Position

  • Knowledge of AmeriCorps/Health Corps member requirements
  • Skill with Microsoft Office or other software for a variety of data processing operations involving a range of problem solving, record keeping, correspondence, and service tracking options.

Supervision

  • The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required
  • The supervisor assigns service activities in terms of project objectives and basic priorities and is available for consultation in resolving controversial issues.

Review

  • The service activities are reviewed for accuracy through spot checks, through complaints from customers, and through observation of the member at service.
  • Completed projects are reviewed very generally to determine that objectives have been met and are in compliance with policies and regulations.

Guidelines

  • Written and oral guides provide specific instructions for doing service
  • Detailed guidelines exist, including regulations and directives, handbooks, precedents, industry standards, and files of previous projects

Complexity

  • The facts or conditions of the service position are clear cut, predictable, and apply directly to the assignment

Special Considerations

  • The member will:
    • Serve approximately 40 hours per week.
    • Serve Monday-Friday from 8:00AM - 5:00PM.
    • NHC members are required to serve onsite 3 times a week from 9am - 5pm, allowing for remote service 2 times per week.
  • Personal vehicles are not required for this position. The clinics are accessible via public transportation.

Language Proficiency Requirements

  • No language other than English is required.

    • Proficiency and skill with Spanish, Cantonese, and/or Mandarin would be useful to the member to communicate with a wide range of people (administrators, teachers, parents, dentists, and children).

Criminal History Check Requirements Beyond NHC Standard Checks

  • Upon Conditional Acceptance of an NHC AmeriCorps Position, all AmeriCorps members must complete TrueScreen Background Checks to be formally placed in the AmeriCorps Program.
  • No Background Checks or Health Exams required beyond what is required of NHC AmeriCorps Members.
