Position Summary

The NHC member will contribute to the operational functions of the Department of Family Medicine and Community Health's (DFMCH) at Penn Medicine. They will help with the administration and operation of community-based mobile clinics including school-based health and wellness programs, immunization programs for seniors, primary and preventive care for individuals experiencing housing insecurity, and other clinics serving special populations (e.g., documented or legal immigrant and legal refugee communities) in West and Southwest Philadelphia, focused on bringing high-quality, low-barrier primary care to populations in need. The member will engage stakeholders to identify, operationalize, and evaluate strategies that result in high-quality programmatic initiatives to meet the health and social needs of the surrounding community. This position will collaborate with the Director of Community Health Services, Division Administrator of Community Health, and Program Manager of Community Health to align programming with needs identified in the federally mandated community health needs assessment and improvement plan. This position is a great opportunity to learn about the health care landscape in Philadelphia, and novel ways of delivering high-quality primary and preventive care in community-based settings.

This role will:

  • Support the administration and operations functions of community health clinics.
  • Build capacity throughout Philadelphia for community health improvement by initiating and facilitating community coalitions and collaboration to address issues concerning our community.
  • Apply performance improvement methodology to develop, implement, and evaluate comprehensive plans that address public health issues within our community.
  • Engage target populations using appropriate age and community-based strategies, striving to remove barriers related to factors affecting health. 

Major Duties and Responsibilities

Capacity Building Responsibilities:

  • Modifying organizational systems/practices to improve service efficiency and/or effectiveness
  • Building relationships with community partners
  • Engaging stakeholders to understand goals, needs, priorities, challenges, and opportunities
  • Developing communications messages/strategies
  • Participating in working groups or other committees to identify gaps in practice or improve systems/processes/workflows

Other major duties:

  • Support daily school-based health clinics and other community clinics on an as-needed basis.
  • Build capacity throughout Philadelphia for community health improvement by initiating and facilitating community coalitions and collaboration to address issues concerning our community.
  • Apply performance improvement methodology to develop, implement, and evaluate comprehensive plans that address public health issues within our community.
  • Outreach to target populations using appropriate age and community-based strategies, striving to remove barriers related to health access and factors affecting health.
  • Manage special projects as assigned.
