Position Summary

The major duties and responsibilities of the Resident Health Navigator is to advocate for quality medical care for 88 individuals who reside at the Scranton Intermediate Care Facility (ICF) facility. In addition, the individual in this position will advocate for meaningful interactions and opportunities within the community. This is done through the preparation for, attendance at and appointment, attending and providing accurate information to the medical provider and then relaying this information back to the appropriate parties at the Center.

In addition, the Resident Health Navigator is responsible for completing the Medical profile that accompanies each individual when they are sent out to the hospital. This profile provides critical information to the hospital and can have a huge impact on the hospital’s ability to accurately and effectively provide care. In this position, the member will accompany individuals to their medical appointment when a nurse is not required. They will ensure that they can provide an accurate representation of the individual’s issues or concerns. They will also relay pertinent information to the nursing and medical records department following the appointment. In addition, the member will complete the medical profile for each resident. This is a form which accompanies individuals to the hospital and provides the essential information that the hospital needs to meet their needs.


The Resident Health Navigator will also explore meaningful opportunities to improve or expand individual’s mental, social and emotional health by investigating resources within the community that enhance an individual’s quality of life.

The hours for this position are Monday through Friday, 8 :30 am to 4:30 pm for 37.5 hours per week. Lunch breaks do not count towards member service hours.

Major Duties and Responsibilities

This position is aligned with the Administrator of Residential Services and includes collaborating with 3 Program Directors: The Director of ICF Resources, The Director of Programs/Social Work, The Director of Medical Records, the Nursing Department, The Direct Care Professionals and Qualified Intellectual Disabilities Professionals. This represents essential program departments involved directly in the care of the individuals served at the Scranton ICF Center.

These individuals all have varying levels of intellectual disabilities and medical needs. The majority of the individuals served are non-ambulatory, and all of these individuals have communication needs. A significant number of these individuals have a diagnosis of cerebral palsy and seizure disorders and require, for the most part, total care.

The Resident Health Navigator will:

-Provide education on how to complete paperwork

-Assist in care coordination and resource referrals

-Track referrals and closing the referral loop

-Evaluate referral charts and obtaining reports

-Conduct extensive and continuous research on social services

 The member will be involved with the residents of St. Joseph's Center. These residents, are medically complex, have various levels of intellectual disabilities and are considered to be vulnerable for those reasons. On a routine basis, this individual will be observing and interacting with the residents as they increase their understanding of each person’s unique needs; physically, socially and emotionally.

Knowledge Required for the Position

Supervision

The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required. With experience, NHC member will progress to more independence in assignments with ongoing consultation and supervision.

Review

Completed projects are reviewed very generally to determine that objectives have been met and are in compliance with policies and regulations.

Guidelines

Detailed guidelines exist, including regulations and directives, handbooks, precedents, industry standards, and files of previous projects

Member has to develop, analyze, or evaluate information before the service position can progress
