Hi everyone! My name is Jazlene Trinidad (she/her). I am currently serving in the National Health Corps SF 2024-2025 term as a Quality Improvement/Diabetes Care Coordinator at HealthRIGHT 360. I take pride in contributing to public health initiatives focused on nutrition, specialty care, and direct patient impact.
Throughout my service, I have had the opportunity to connect with many inspiring physicians and healthcare professionals. Today, I want to spotlight Chloe Reagan, a Physician Assistant who recently celebrated her second year of residency. She is deeply admired by patients for her passionate and empathetic care. At HealthRIGHT 360, she works with an array of patients, making a meaningful impact every day.
Our clinic is in the heart of the city, and we find that patient interactions and their progress are the most rewarding aspects of our work. We are committed to improving healthcare outcomes for all.
Chloe embodies excellence—her strong work ethic and involvement in various organizations reflect the kind of dedication we value at NHC SF. She previously served in the National Health Service Corps (NHSC), a program under the Health Resources and Services Administration (HRSA). She’s not new to this—only true to this!

What is your name and where is your hometown?
My name is Chloe Reagan, PA (she/her), I am from Iowa City, Iowa.
What is your role at HealthRIGHT360? How long have you been at the organization?
I am a physician assistant, and APP resident. I have been practicing since September 2023.
Knowing that you were a National Health Service Corps member, how has that prepared you for your career?
I have always wanted to work with the under-served because I feel this population has less access to needed medical resources. Receiving the NHSC scholarship has allowed me to work with this population and learn more about the barriers to healthcare that many people face. It has allowed me to learn about more resources for my patients and how to advocate for people who often have a difficult time doing it for themselves. Working at HealthRIGHT 360 has also allowed me to expand my knowledge of medicine and learn about complex disease processes and treatment.
What keeps you motivated to help patients every day?
Knowing that the population of patients we see at HR360 need high-quality healthcare motivates me every day to do my best with each patient and keep learning daily to better assist my patients with their needs.
Are there any setbacks you face regularly in your field of work?
Access to care can be difficult especially when it comes to specialty referrals. It often takes longer than preferred to see a specialist, and sometimes we will get notification that a site is no longer taking new patients.
Do you have any advice for anyone interested in joining a similar NHC or AmeriCorps program on the pre-med track?
My advice would be to focus on the success stories and the patients whose lives have changed due to having better access to medicine.
Any hobbies, interests, or anything from your background that you'd like to share?
On my off time I like to hike with my dog and loved ones. I also love to explore new areas and restaurants in the bay!
Many future healthcare providers gain invaluable experience through programs like NHC, AmeriCorps, and NHSC. These programs also provide financial support and professional development opportunities for those pursuing medical school or other healthcare careers. The sky is the limit when you apply yourself to programs like ours!
