Position Summary
Member will do outreach and complete screening tools and enrollment paperwork with client that come into the clinic (Life Tree) in 32210 zip code. Member will also, provide resource information for needed services.
Major Duties and Responsibilities
- Developing and/or facilitating classes with other professionals
- Maintaining professional boundaries and respecting patient confidentiality
- Attending and participating in community or host site events
- Providing social service navigation
Outreach, completing enrollment paperwork and screening tools as well as providing resource information
- Providing education to clients and referring them to providers
- Providing health education to at-risk populations
- Attending trainings and/or community events
- Conducting enrollment benefits screenings

Knowledge Required for the Position
Knowledge of AmeriCorps/Health Corps member requirements
Supervision
The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required
Review
Completed projects are reviewed very generally to determine that objectives have been met and are in compliance with policies and regulations.

Personal vehicle required, Significant travel (more than 25%)
