Position Summary
The member will provide a variety of direct social and education services to high-risk LGBTQIA+ young adults and will engage
in building and maintaining community partnerships to expand the offerings provided to LGBTQ+ young adults at JASMYN.
They will do this through their support of JASMYN’s ACCESS program focusing on providing resources for unstable and
unhoused young adults, one on one housing assessments, health screening, health education programming, and wrap-around
care coordination services to support LGBTQ+ young adults with needs identied through intakes and assessments.
The ACCESS program that provides access to basic wrap around services including: showers, food pantry, hot meals, laundry,
computer lab and one on one support. At access the member will also conduct housing screenings, health screenings, risk
behavior assessments, safety planning and enrollment into JASMYN’s intensive case management services for youth
experiencing homelessness, substance use, legal and mental health issues. The member will serve one on one providing direct
services to JASMYN youth during ACCESS. They will work with supervisors to create outcome measures for ACCESS that will
help streamline internal enrollments into case management, create a community resource guide to improve awareness of local
resources available in the community. The member will focus on developing working relationships with young adults
accessing JASMYN and providing individual support through housing screening, health assessments, membership
enrollments, risk screening and resource linkage.
The member will also create capacity for expanded health education, life skills education and access to primary care services
at JASMYN through the development and implementation of workshops in topics such as healthy eating, healthy body image,
gender and sexual identity, substance use and abuse topics, healthy relationships, and sexual health. Education sessions can
also address life skills such as job readiness workshops, nancial stability, etc. While some of these sessions will be formally
scheduled, it is anticipated that many of these short educational sessions using conversations and online tools will be
presented in “teachable moments”, when the topics are most relevant to the immediate needs and questions that arise in the
friendly conversation of the youth center. The member will work to enhance access to and enrollment in health care services
through JASMYN's partnership with Telescope Health. The member will also provide limited follow up care coordination
services after enrollment as needed.
JASMYN’s member will be invited to participate in all staff activities and trainings to address structural oppression in policies,
procedures, and practices. This includes but is not limited to fully participating in JASMYN’s working meetings that address
embedding racial equity in our organization, participating in professional development training with staff that address racism
in health disparities and participating in team meetings and youth programs that provide opportunities to have open
discussions about structural racism.
Major Duties and Responsibilities
- Practicing intake and referrals of patients in need
- Providing follow-up and case management to deliver assistance and information
- Maintaining professional boundaries and respecting patient confidentiality
- Collaborating with supervisors and other leadership
- Conducting social determinants or health screener
Participating in JASMYN team meetings, housing team meetings and participating in JASMYN team building, trainings and
DEI initiatives.

Knowledge Required for the Position
Skill with Microsoft Office or other software for a variety of data processing operations involving a range of problem solving, record keeping, correspondence, and service tracking options.
Supervision
The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required
Review
Completed projects are reviewed very generally to determine that objectives have been met and are in compliance with policies and regulations.
Guidelines
Written and oral guides provide specific instructions for doing service, Guidelines are generally applicable, but the member independently makes adaptations in dealing with problems and unusual situations
Complexity
Member has little or no choice about how to perform the service position, The facts or conditions of the service position are clear cut, predictable, and apply directly to the assignment

Criminal History Check Requirements Beyond NHC Standard Checks
The background that NHC provides to JASMYN to become an applicant is sufficient for JASMYN. WE do require a criminal background check for all employees, as required by some of our funders, but the existence of a criminal background is not a barrier to employment.
